Caremark (Plymouth) are looking for an Evening and Weekend On Call Care Coordinator working from home to join our friendly team.

Positions Available Evening and Weekend On Call Care-Coordinator on a rolling rota pattern weekday evenings 17:30-22.30 Every other weekend 06:30- 22:30

Week One - Thursday Friday Saturday, Sunday.

Week Two - Tuesday Wednesday Thursday

We are looking for a Part Time Out of Hours Care Co-ordinator. You must have the ability to work independently, maintain confidentiality of information, meet deadlines and effectively multi-task.

Provide out of hours telephone assistance for all issues arising from customers or care workers.

· Arranging alternative care staff to provide care if someone becomes unavailable (e.g. due to staff illness)

· Liaise with the other co-ordinators for weekly hand overs of new customers and care workers.

· Update the rostering system and Pass System with changes out of office hours.

· Give direction to care workers in line with policies and procedures when issues arise and follow company policies and procedures to deal with emergencies.

· Undertake any other actions that may be required during out of hours to support the business and maintain quality for customers.

· Liaise with Adult Social Care during out of hours for any issues relating to local authority care packages.

· Liaise with family and other care professionals as required to resolve customer issues.

Salary: £11,790.00-£12,840.00 per year
